Services:

  • Change of Address or Name (required by law within 10 days).
  • Renewal and Expired (every four years - 30 days from birth date. Written and eye exams must be taken if license is not renewed within one year of expiration at Bowman Field location).
  • Replacement of lost license or I.D. (two documents with signature required).
  • Personal Identification Card (requires birth certificate, signed social security card, another document with signature, and proof of residency). If under 18, parent or legal guardian must accompany. 
  • Permit Testing - Requirements for obtaining a permit/license:
    • Must be at least 16 years of age
    • If under the age 18, a parent or legal guardian must sign the application
    • Written, vision, and skills testing are required for all new drivers
    • Birth certificate (photo copies are not accepted
    • A social security card (laminated social security cards are invalid and cannot be accepted)
    • If under the age 18, must present a School Compliance Form obtained from the student's school district of residence. (No Pass / No Drive Law - KRS 159.051 effective 8/1/2007)
  • Out of State Transfers (Valid out-of-state license, social security card required and proof of residency. Written and eye tests required only if license is expired at Bowman Field location).
  • Reinstatements (Department of Transportation has sole authority to reinstate a suspended drivers license. They are located at 4109 Bardstown Road, 502-493-1477).
  • Non-US Citizen Applicants (must have documents authenticated by Department of Transportation (4109 Bardstown Road, 502-493-1477) before applying at Bowman Field location.
  • Commercial Drivers License (Kentucky State Police administer testing at Bowman Field location only) Traffic School (cannot enroll if attended within the last 12 months). Call 502-564- 6800 for information.

State Police Testing Hours: Monday 8 a.m. - 5:30 p.m., Tuesday - Friday 8 a.m. - 3:30 p.m. for permits. Road test requires appointment by calling (502) 454-3327.
